This form is required for any overnight school trips. 

Permission for Prescription Medications Taken at School Form

Prescription Medication Form: This form must be completed by your child's PHYSICIAN if prescription medicine has to be administered your child is in school. 

Note: If your child/children require medications they must carry on them at all times - please contact the Health Office directly. 

Well Exam - Sports Participation Clearance Form

The Sports Participation Clearance Form is needed in order for students to play sports.
